Diabetes Connect

Diabetes Connect is an exciting new program being rolled out by GPQ in collaboration with divisions of general practice and Medicare Locals, under the Queensland government’s recent investment in prevention, early intervention and management of Type 2 diabetes.

Diabetes Connect aims to increase the capacity of general practice to meet the complex needs of patients with diabetes and to enhance care and communication pathways between General Practitioners and other key health and community service providers.

The Diabetes Connect initiative has two main components: Connecting Professions and Connecting Online.
