Diabetes Connect
Diabetes Connect: Connecting Professions; Connecting Online has been made possible through funding from Queensland Health under the Diabetes Action Plan announced in 2010. This major project saw the implementation of online learning modules coupled with access to interprofessional learning workshops to develop localised solutions to better support the prevention and management of patients at risk of, or with, diabetes.
As the funding period is now over, CheckUP is currently exploring options to sustain, as well as expand, access to the Diabetes Connect learning package for health care professionals within Australia.
